What We Collect

When you submit our contact form, we collect:

• Message content
• Date and time of submission

The partner portal uses essential cookies to maintain your sign-in session and portal preferences. Our application does not include advertising pixels or Google Analytics tags. Hosting services may process technical information such as IP addresses, browser details, and requests for security and operation of the site.

Playing an embedded webinar connects your browser to Vimeo. Following enrollment links takes you to DocuSign. These services have their own privacy policies. Please do not enter patient information into public inquiry forms or the website assistant.

How We Use Your Information

We use contact inquiry information to:

• Provide information about ACCESS programs relevant to your request
• Follow up on your expressed interest

What We Do Not Do

• We do not sell, rent, or share your personal information with third parties for marketing purposes.

Data Storage

Form submissions are transmitted to a third-party form processing service (SheetMonkey) and stored in a private spreadsheet accessible only to authorized ACCESS team members. Data is retained for as long as necessary to address your inquiry and maintain business records.

Partner Portal and AI Assistants

Portal accounts, center affiliations, agreements, uploaded documents, reporting records, and payment records are processed to operate ACCESS programs and administer access. The portal uses Supabase for authentication, database, and file storage, and Resend for transactional email. When you use an AI assistant, your question and relevant information permitted for your access level are sent to OpenAI to generate an answer. AI answers require human review.

Your Rights

You may request access to, correction of, or deletion of any personal information we hold about you by contacting us at the email address below. We will respond within 30 days.
